Reablement care is a type of home care that is designed to aid the process of adjustment and recovery following a stay in hospital or after an illness or injury. Unlike normal home care which provides assistance to those that may have lost some independence with their daily routines, reablement care is aimed at encouraging and supporting independence during the recovery process.
A reablement care plan is designed to support someone through their transition from hospital back to living in their own home. Reablement carers are specifically trained with the skills to help encourage independence safely as well as recognising the emotional effect that an injury or illness can have.
Through reablement care, a person is given the support they need during the recovery process to allow them to get back to living independently with expert support.
The length of reablement care will vary depending on your illness or injury and the support needed to help you get back to normality. Most people will need this type of care for one or two weeks, but it could be up to six weeks or more, but we can work with you as long as you need us.
Opting for reablement care provides you with the help you need to regain your independence and get out of hospital sooner – avoiding unnecessary hospital stays or the risk of rehospitalisation. It means you can get back to your own home and family sooner and back to some form of normality. There is also scientific evidence that many patients heal faster in the comfort of their own home.
During your period of reablement care, you’ll receive support to do ordinary activities such as washing, dressing, cooking, getting around your home and even out and about. We’ll work with you to get to milestones together and be with you every step of the way.
